Who May Request Coverage
All ASRT members in good standing who are residents of the US under age 60, actively working (at least 30 hours per week) and not in full-time military service may apply for up to $5,000 per month in benefits. Members age 60 through 64 may apply for up to $500 per month, and may only elect either a 90 or 180-day waiting period.

Features and Benefits
You can select to receive anywhere from $100 to $5,000 a month, subject to income ratios. Total benefits you receive from this plan and from any other income replacement plans (including Worker's Compensation, Social Security, employer-sponsored salary continuation, group or franchise plans or retirement programs) may not exceed 60% of your basic monthly pay.

Waiting Period
The choice is yours?benefits can begin after 30, 90 or 180 days of disability. Choose a waiting period that best suits your needs by comparing your current savings versus your expenses. If your savings can only meet one or two week?s worth of expenses during a disabling illness, then select the 30-day waiting period. However, if you feel that you can meet your expenses for a longer period of time, then choose a 90- or 180-day waiting period.

Benefit Period
PLAN I...if you are totally disabled by a covered accident or illness prior to age 60, you will receive benefits beginning on the day after your chosen waiting period ends, and continuing until you reach age 65, or 2 years, but not beyond age 70 if disabled on or after age 60.

PLAN II... if you are disabled by a covered accident or illness, prior to age 60, you will receive benefits beginning on the day after your chosen waiting period ends and continuing for up to five full years, or 2 years, but not beyond age 70 if disabled on or after age 60.

Rehabilitation Benefit
This provision helps disabled individuals return to the work force. The Insured and the insurance company must mutually agree upon a rehabilitative program for the insured that becomes Totally Disabled. Monthly benefits will continue during the insured's rehabilitation subject to the terms of the agreement.

Partial Disability Benefit
You can receive a portion of your chosen monthly benefit if, because of an illness or injury, you are unable to perform the material duties of your regular job but you are able to perform: at least one of these duties on a part-time basis, or at least one, but not all, of these duties on a full-time basis. To be considered partially disabled, you must also be under the regular care of a physician and have a loss of monthly income of at least 20%. Partial disability benefits will be paid up to your maximum benefit period according to the formula listed on the schedule of benefits.

Benefits for Mental, Nervous, & Emotional Disorders, Alcoholism & Drug AddictionsM
If total disability is due to alcoholism, drug addiction or a mental, nervous or emotional disorder, a maximum of 24 monthly benefits will be paid while such disability continues.

When an individual becomes disabled, they are helped with rehabilitation and motivation to return to work as soon as reasonably possible.

Survivor Benefit
If the insured is totally disabled for more than 180 days, is receiving benefits for the disability immediately prior to their death and provided the maximum payment period has not been met, a special payment equal to three times their selected monthly benefit will be paid to their spouse, if living. If not living, it will be paid in equal shares to their living children or, if there are none, to their estate.

Definition of Total Disability
TOTAL DISABILITY means the complete inability of the person to perform the material duties of his regular occupation; "his regular occupation? is that which the person was performing on the day before total disability began.

The total disability must be a result of an injury or sickness. To be considered totally disabled, a person must also be under the regular care of physician, and must not be performing the duties of any gainful job.

Waiver of Premium
If you become totally disabled prior to age 60 and receive benefits under the policy for six consecutive months, you will not have to pay any premiums for your Disability Income Insurance coverage. Premium payments will resume when you are no longer totally disabled and, stop receiving monthly benefits.

When Insurance Becomes Effective
Insurance will take effect on the first of the month coinciding with or next following the date of approval of your application, provided the required premiums are paid.

You must be actively at work on the date your insurance is to take effect. If you are not, such insurance will take effect on the day you resume such work. The date insurance is to take effect might not be a scheduled work day. If so, you will be considered actively at work on such date if you were actively at work on your last scheduled workday.

When Coverage Ends
Coverage will continue until: The date the policy ends; the date insurance ends for your class; the end of the period for which the last premium has been paid by you; the date you cease to be actively at work on a full-time basis for reasons other than total disability; the premium due date coinciding with or next following the date you attain age 70; the date that you are no longer a resident of the United States; the date you have been on foreign travel for longer than 3 months; the date you enter active military duty; or the date you cease to be a member in good standing with ASRT.

Exclusions And Limitations
No benefits will be paid for any period of disability during which you are not under the direct care and treatment of a licensed physician. No benefits will be paid for any disability due to: Intentionally self-inflicted injury; war or acts of war; confinement in a penal institution; or committing a crime or an attempt to do so.

Pre-Existing Conditions Limitation
Plan II Only - A pre-existing condition is an injury or sickness which you have incurred charges, received medical treatment, consulted a physician, or taken prescribed drugs during the 12-month period immediately preceding the effective date of your insurance. Disabilities caused by pre-existing conditions will not be covered until you have not incurred charged, received medical treatment, consulted a physician, or taken prescribed drugs for that condition or complications of it for 12 consecutive months or you have been insured for 24 months.

Successive Periods of Disability
Successive periods of disability will be considered one period of disability unless they are due to unrelated causes, or separated by a return to work for 6 or more continuous months. A separate waiting period will apply for each separate period of total disability.

Medical Information Bureau (MIB) Disclosure Notice
Information regarding your insurability will be treated as confidential. The United States Life Insurance Company in the City of New York, or its reinsurers may, however, make a brief report thereon to MIB, a not-for-profit membership organization of insurance companies, which operates an information exchange on behalf of its Members. If you apply to another MIB member company for life or health insurance coverage, or a claim for benefits is submitted to such a company, MIB, upon request, will supply such company with the information in its file.

Upon receipt of a request from you MIB will arrange disclosure of any information it may have in your file. Please contact MIB at 866-692-6901 (TTY 866-346-3642). If you question the accuracy of information in MIB?s file, you may contact MIB and seek a correction in accordance with the procedures set forth in the Federal Fair Credit Reporting Act. Your total premium will include a $2.00 processing fee. Premiums increase on the premium due date occurring on or next following the date you enter a new age category.

Reduction Formula for monthly benefits amounts in excess of $2,000

On your premium due date coinciding with or next following the date you attain age 65 your monthly benefit will be reduced to a maximum of $2,000. This reduction applies only to total disability commencing on or after the effective date of change.
