Term Life Insurance is pure insurance - without any risky investment or savings frills that increase your costs. It provides the highest level of financial protection for your loved ones at a price you can afford. ASRT members can apply for coverage from $100,000 to $1,000,000 in increments of $1,000. Spouse coverage is also available up to $1,000,000 not to exceed the member's benefit level.

Who May Request Coverage
You may apply for up to $1,000,000 in coverage, for yourself and for your spouse as long as you are a member in good
standing of the ASRT, under age 65 for the 10-year plan and under 55 for the 20-year plan. Simply complete and return
the enclosed Application. In most cases, evidence of insurability may be requested. The Member must be actively at
work on the date the insurance is to take effect. Otherwise, the insurance will take effect on the date of return to
work.
Features and Benefits
A terminal illness could be extremely costly with significant financial ramifications. This Level Term Life Plan
includes an Accelerated Death Benefit to help alleviate the financial challenges that often accompany terminal illness.
If you are diagnosed as having 6 months or less to live, and not expected to recover, you can collect 60% of your
selected coverage amount to spend as you see fit. The Living Benefit will become effective after a 180-day waiting
period. The remaining balance of your benefit amount will then be paid to your beneficiaries upon your death.
Receipt of Living Benefits may be taxable. Please consult your personal tax advisor for guidance. Living Benefits will
not be paid if: you made an absolute assignment of your life insurance; all or part of your insurance is to be paid to
your children or former spouse as part of a court-approved divorce agreement; United States Life doesn't receive
written consent by an irrevocable beneficiary; or the terminal illness is a result of intentional self-inflicted injury
or attempted suicide.
Attractive Rates
Rates remain level for the 10 or 20-year term based on your age of entry into the plan. Rates are not guaranteed and
may be adjusted on the plan's anniversary date upon approval of the insurance company and ASRT. There are preferred,
standard and smoker premium rates for face amounts over $200,000. The rate you qualify for is determined by your
medical history and current health status. Premiums vary based upon age, plan selected and amount of insurance you
choose.

Beneficiary
This versatile Plan allows you to choose any beneficiaries: your spouse; child or children; partner or professional
corporation; trust or trustee. You may also transfer Plan ownership to your spouse, professional corporation or other
third party. You can change your beneficiary, unless irrevocable, at any time by writing to the Plan Administrator.
Your beneficiary can receive benefits as a lump sum payment, in monthly payouts or through trust administration.
When Insurance Becomes Effective
Insurance will take effect on the first of the month coinciding with or next following the date of approval of your
application, provided the required premiums are paid.
When Coverage Ends
Coverage will end when the member reaches age 75 or: 1) the date the group policy ends at the option of United States
Life or the Policyholder; 2) the date insurance ends for your class; 3) the end of the period for which the last
premium has been paid for you; 4) your premium due date coinciding with or next following the date you cease to be
actively at work; 5) your premium due date coinciding with or next following the date you cease to be a member in
good standing; or ; 6) the date you enter active military duty.
Your Coverage Is Renewable
Once you are insured, you can renew your coverage with no reduction in your benefit level up to age 75 regardless of
changes in your health, as long as you maintain your ASRT membership, pay your premiums when due, and the Group
Policy remains in force.
Exclusions And Limitations
Suicide is typically not covered during the first two certificate years. After two years, your coverage becomes
incontestable, except for non-payment of premiums.

Medical Information Bureau (MIB) Disclosure Notice
Information regarding your insurability will be treated as
confidential. The United States Life Insurance Company in the City of New York, or its reinsurers may, however,
make a brief report thereon to MIB, a not-for-profit membership organization of insurance companies, which operates
an information exchange on behalf of its Members. If you apply to another MIB member company for life or health
insurance coverage, or a claim for benefits is submitted to such a company, MIB, upon request, will supply such
company with the information in its file.
Upon receipt of a request from you MIB will arrange disclosure of any information it may have in your file. Please
contact MIB at 866-692-6901 (TTY 866-346-3642). If you question the accuracy of information in MIB's file, you may
contact MIB and seek a correction in accordance with the procedures set forth in the Federal Fair Credit Reporting

The United States Life Insurance Company in the City of New York, or its reinsurers, may also release information
in its file to other insurance companies to whom you may apply for life or health insurance, or to whom a claim for
benefits may be submitted.

When you receive your certificate, examine it carefully. If you are not completely satisfied with your 10 or
20-Year Level Term Life Insurance Coverage, simply return your certificate within 30 days, with your request for a
full refund. No questions will be asked and there will be no further obligation.
